Plain summary: This tool lets you record a hockey head-impact history and current symptoms so you can discuss them with a doctor. If you create an account, that information is stored securely so you can track it over time. We never sell or share it, and you can delete everything yourself at any time. This is not a medical device and not a diagnosis.

If you use the app without an account, your information stays on your device and is not sent to us.
Only to provide the tool: to let you save the record, return to it over time, and produce a one-page summary to bring to a doctor. We do not use it for advertising.
Account and saved data are stored with our backend provider (Supabase) using encryption in transit and at rest, with access controls so that only you can access your own records.
We do not sell, rent, or share your personal information. We use a service provider (Supabase) solely to store your data on our behalf. We may disclose information only if required by law.
We keep your saved data until you delete it or your account.
This app is intended for adults 18 and older. It is designed to be used by an adult former player, or by a parent or legal guardian recording information about a minor athlete in their care — not by children directly. Children should not create accounts or use the app themselves. We do not knowingly collect personal information directly from a child under 13. When a parent or guardian records a minor athlete’s information, they do so as the adult account holder, are responsible for that information, and can view or permanently delete it at any time inside the app. If we learn we have collected personal information directly from a child under 13 without a parent or guardian, we will delete it. We never sell a child’s information or use it for advertising. To review or delete a minor’s information, contact support@continuumimprovement.com.
This app documents self-reported head-impact history and symptoms to support a conversation with a clinician. It is not a medical device, does not diagnose, and cannot tell you whether a brain injury or CTE is present. CTE can only be diagnosed after death.
